1967 Sears Suburban Fork Lift Conversion

I believe the tractor is a 1967 but have no information on the Fork Lift Conversion. Any info that can be provided would be most appreciated. I can this a conversion vs. an attachment because this is intended to be a permanent installation as the steering is modified to turn the wheels reverse of the original direction to get rear wheel steering.
